Clinical Triage
Technical Triage and Clinical Triage come together to form a full triage process.
Once an out of threshold alert is confirmed, our trained nurses contact the patient and follow an agreed process in order to follow up on the questions from the monitor, and work with the community nursing teams, GPs and secondary care organisations to ensure appropriate action.
Results of the Triage Process
All patient interactions are logged on the computer, and on the home monitoring system database.
Additionally all telephone calls are recorded to assure an accurate audit trail. This enables Alere and the trusts to see detailed reports for each patient and also to analyse the effectiveness of the service.
